PGG - Rotation Evaluation Form

Please complete the mandatory evaluation of the named student who has rotated through your laboratory. It is recommended that you discuss identifiable strengths and weaknesses with the student. Upon completion of the evaluation, you must recommend an assigned grade. In recommending the grade you should consider your evaluation under each of the criteria listed. It is recommended that you make use of the entire grading system in recommending a grade. An “A” grade should only be assigned if the performance is outstanding in each of the areas listed.
