iPad Policy Agreement
Perelman School of Medicine will provide iPads to all incoming first year medical students. This agreement establishes the following expectations and guidelines for iPad use in the pre-clinical curriculum:
1) Students must use this device in a responsible manner and in accordance with all University of Pennsylvania policies.
2) Students may use the device for both personal and academic functions.
3) Students must return the iPad to SOM when requested. iPad privileges may be rescinded and the device confiscated in cases where students are found not to have followed policies and guidelines for appropriate use of the device.
4) Students must, at all times, take appropriate steps to protect the iPad and data against loss or theft. The device should never be left in public places, should not be checked in with luggage when traveling, and should not be left in vehicles unless the vehicle is locked and the iPad is hidden from view.
5) iPads are not to be loaned, even temporarily, to anyone else. It is for your use only while enrolled at the School of Medicine.
6) Students must immediately report the loss, damage or theft of an iPad to Mark Malachesky (Mark.Malachesky@pennmedicine.upenn.edu).
7) If the iPad is reported lost or stolen its entire contents will be wirelessly erased from the device.
8) Students must protect the data on the iPad with a password and auto-lock timeout and follow all University security requirements.
9) Students must accept financial responsibility for the loss or theft of the device and the disclosure of information resulting from failure to take appropriate steps to protect the iPad.
10) Every iPad comes with one year of hardware repair coverage through its limited warranty and up to 90 days of complimentary technical support. Optional AppleCare+ extends your iPad coverage and includes unlimited incidents of accidental damage protection. Each incident is subject to a service fee of $49 for iPad plus applicable tax. In addition, you’ll get 24/7 priority access to Apple experts by chat or phone. You will be responsible for getting AppleCare+ on your own within the 30 days. 11) Apple strongly cautions against installing any software that hacks iOS. It is also important to note that unauthorized modification of iOS is a violation of the iOS end-user software license agreement and because of this, Apple may deny service for an iPhone, iPad, or iPod touch that has installed any unauthorized software. https://support.apple.com/en-us/HT201954
12) Students must frequently make encrypted backup copies of iPad content in the case of loss or data corruption.
13) Under no circumstances can Protected Health Information (PHI) regarding any patients, including your LEAPP patient, be stored on the iPad.
14) PHI may never be stored on iCloud. iCloud is for personal use only.

Penn students may only use Perelman School of Medicine course materials as intended for curriculum and course---related purposes. These materials are copyrighted by the University or others. Access to this content is for personal academic study and review purposes only.
Unless otherwise stated in writing, students may not share, distribute, modify, transmit, reuse, sell, or disseminate any of this content.
Restricted Data and HIPAA Compliance:
Students must protect their iPads by following Perelman School of Medicine mobile device security guidelines (especially by having a security passcode set and encrypting the iPad backup) to protect any Penn Confidential Information that may be accessible by this device. Students must not access or store Penn confidential information on the iPad. iPads are not intended for the storage of Restricted Information, specifically including Protected Health Information (PHI).
University of Pennsylvania Computer and Network Usage Policy:
Students must respect copyrights and licenses, respect the integrity of computer--- based information resources and refrain from seeking to gain unauthorized access, and respect the rights of other information resource users.
